Onboarding Steps

Please reach us at tfisher@rebuildingmentalwealth.com if you have questions or concerns. 

  • After scheduling your first appointment, you will receive an email with links to our onboarding forms. These include medical history, consent forms, and any additional information needed for your care.
  • Please complete these forms at least 48 hours before your appointment to ensure your provider has time to review them.


  • If you’re using insurance, we will verify your coverage ahead of your visit. Please provide a copy of your insurance card during the onboarding process.
  • If you’re a self-pay client, payment arrangements will be discussed prior to your appointment.


You will receive an appointment reminder via email or text 24-48 hours before your scheduled time. This will include the date, time, and location of your appointment, as well as any additional instructions.


Arrival Procedures for Your First Appointment

  • A valid photo ID.
  • Your insurance card (if applicable).
  • A list of current medications and dosages.
  • Any relevant medical or mental health records.


Please arrive 10-15 minutes early for your first appointment. This allows time for check-in and to address any last-minute questions about your forms or insurance.


  • Upon arrival, check in at the front desk and confirm your information.
  • If your appointment is virtual, please log in to the secure telehealth platform at least 5 minutes early to ensure your connection is working properly.


  • During your first session, your provider will review your medical history, discuss your concerns and goals, and work with you to create a personalized treatment plan.

This is your opportunity to share your story and ask questions. The more your provider knows, the better they can tailor your care.


We’re here to support you, and our goal is to create a comfortable and welcoming environment.


If you’re unsure about anything, don’t hesitate to ask. Your provider will gladly clarify any part of the process.
